Continuum of Care

A Continuum of Care (CoC) is a regional or local planning body that coordinates housing and services for homeless families and individuals. This planning body works toward a single application for a U.S. Department of Housing and Urban Development McKinney-Vento Homeless Assistance Grant that requires the identification of four main parts of a housing continuum:

  • Outreach, intake, and assessment to identify service and housing needs for homeless persons.
  • Emergency shelter to provide an immediate and safe alternative to sleeping on the streets.
  • Transitional housing with supportive services to allow for development of skills that will be needed once permanently housed.
  • Permanent and permanent supportive affordable housing for individual and families with services if necessary.

The Supportive Housing Program

The Supportive Housing Program (SHP) provides funds to develop supportive housing - including the acquisition or construction of housing - and supportive services to assist homeless persons in the transition from homelessness to independent living.

The Shelter + Care Program

The Shelter Plus Care (S+C) program provides rental assistance and supportive services on a long-term basis for homeless persons with disabilities such as serious mental illness, chronic problems with alcohol and/or drugs, and AIDS or related diseases and their families. The program allows for a variety of housing choices, and a range of supportive services funded by other sources, in response to the needs of the hard-to-reach homeless population with disabilities.

In Alaska, these programs are administered through Alaska Housing Finance Corporation.
